Change of Ownership

To help assist you in making changes to your account, we have listed situations below that require changes to be made to your account. Next to each situation we have listed the documentation that we will require in order to complete the changes. All change requests must be submitted in writing and cannot be taken over the phone.

Address Change	Submit request in writing with your signature
Name Changes	Marriage Certificate or Divorce Decree reinstating maiden name or other legal documentation of name change
Company Name Change	Certificate of Name Change including tax I.D.
Company Name Change Due to Merger	Certificate of Merger including tax I.D.
Appointment of Attorney-In-Fact	Copy of Power of Attorney
Sale of Interest	Conveyance filed of record in the county/parish in which the property is located
Ownership Change Due to Divorce	Complete Divorce Decree including Settlement Agreement and recorded Conveyances (if appropriate)
Trust or Partnership Creation or Dissolution	Trust or Partnership Agreement and recorded Conveyance(s) (if appropriate)
Changes in Trustee	Instrument by which the Successor Trustee was Appointed
Joint Tenancy with Death of One Tenant	Death Certificate of Deceased Tenant
Death of Owner with a Will with Probate Proceeding Conducted in State Where the Property is Located	Death Certificate Order Admitting Will to Probate Letter Testamentary Last Will and Testament Evidence Debts and Taxes have been paid Final Decree of Distribution or Judgment of Possession
Death of Owner with a Will with Probate Proceeding Conducted Outside the State Where the Property is Located	Death Certificate Order Admitting Will to Probate Letter Testamentary Last Will and Testament Evidence Debts and Taxes have been paid Final Decree of Distribution or Judgment of Possession Documentation of Ancillary Probate Proceedings opened in all affected states or Affidavit of Heirship
Death of Owner Without a Will	Death Certificate Letters of Administration Affidavit of Heirship Evidence Debts and Taxes have been paid
Life Estate - Death	Life Tenant's Death Certificate Names and Addresses of Remaindermen
Bankruptcy - Chapter 11	Plan of Reorganization Order confirming the Plan of Reorganization
Bankruptcy - Chapter 7	Recorded Conveyance (as appropriate) Order Naming Trustee of Bankruptcy Estate
